Players documentation MUST be provided on the first night of practice, Wednesday, June 15th.
Every player selected for all-star play must present the following documents to the all-star manager on the date the teams are officially announced. Parents should check their birth records now to make sure they qualify for acceptance:
1. Original Birth Certificates (with raised seal)
2. If child’s last name is different from parent’s last name, copies of records establishing a parental or guardian relationship between the parent and player
3. Three copies of documents from the following categories of records (no two documents can be from the same category); these records must show your current home address:
Driver’s License
Voter’s Registration
School records (must have home address on the record)
Welfare/child care records
Federal records
State records
Local (municipal) records
Support payment records
Homeowners or tenant records
Utility bills
Financial records
Insurance documents
Medical records
Military records
Internet, cable or satellite bills/records
Vehicle records
The easiest documents to provide tend to be: parents drivers license (make sure it has valid district address), vehicle registration and a utility bill dated before February 1, 2011.
Copy all 3 proofs onto one page.
